Re-Entry Coordinator
The Re-Entry Coordinator will work alongside the social services team to support recently incarcerated community members, with a focus on transgender women, to assist in navigating the connections necessary to securely transition back into society through coordinating intakes, ISP’s, referrals, skills building and educational life skills groups, and coalition building with key stakeholders. The coordinator will serve as the first point of contact for clients interested in
participating in re-entry services through HIPS RESTART Program.
PROGRAM RESPONSIBILITIES
● Create recruitment and informational materials for HIPS re-entry services including digital and print materials and talking points for staff
● Interviews incoming clients to determine their needs/goals and develops an individualized plan. This assessment will include documents that the client will need to secure public assistance, social services or education needs. The assessment will include any requirements to reunite with their children/families.
● Maintain accurate and up to date intake records on the status of each client.
● Supports the clients with goal completion through skills-building activities, linkages and referrals, and acting as a client advocate.
● Coordinates transportation for clients.
● Develop skills building group agendas and activities.
● Builds new and strengthens existing relationships with community partners including CBO’s, job training programs, potential employers, faith based leaders and institutions.
● Participates in regular meetings with the care team to facilitate problem-solving and to improve communication when needed.
● Coordinates client level data entry and maintenance (i.e. appointment tracking, length of stay, milestones, etc.)
● Work to build the trust and confidence of clients;
● Work with all HIPS staff members in assisting clients identifying services to address other aspects of their life that affect their health, such as mental health and substance abuse treatment, housing programs, and education and employment services;
● Submit clear and accurate data collection forms for all work performed;
● Comply with all program protocols, including those regarding confidentiality of client information;
● Complete employee orientation and all mandatory program trainings to guarantee appropriate competency in program requirements;
● Participate in all staff and program meetings, site visits with funders, required network events, and mandatory trainings;
● Keep manager informed of all problems, challenges and conflicts related to the job. ADDITIONAL RESPONSIBILITIES
● Attend staff meetings, one-on-one supervisions and staff development days to share information and plan program work;
● As part of a team, share in other organizational responsibilities as required, including general administrative work and volunteer training;
● Attend appropriate local, regional and national meetings to seek out current information about transgender programs, harm reduction, re-entry programs
KNOWLEDGE AND SKILL REQUIREMENTS
● Knowledge of local communities of drug users, sex workers, and transgender individuals.
● Passion for the mission of HIPS and working from a Harm Reduction philosophy, including a strong commitment to working to improve the health and quality of life of people living in street economies
● Knowledge of trauma informed care guided programming
● Knowledge of DC community resources/services is important, particularly in Wards 5-8.
● Ability to assess clients for needs related to treatment education, risk reduction and prevention.
● Experience with/understanding of HIV/HCV risk factors, including homelessness, incarceration, sex work, and/or substance use.
● Strong communication skills are essential.
● Ability to conduct sensitive, empathetic interviews and conversations that respect the dignity and diversity of clients.
● Ability to engage staff and clients in a supportive and empowering way while also managing interpersonal conflict.
● Ability to maintain client records and information in an accurate, timely and confidential manner.
● Sound practical judgment; the ability to work independently during periods of less supervision; the ability to work effectively in a busy office; and the interpersonal skills needed to work effectively in partnership with other HIPS staff members.

Harm Reduction Educator
The Harm Reduction Program Educator will assist in the service provision of all daily harm reduction activities in the day center located at The New York Avenue Presbyterian Church. The address is 1313 New York Ave NW Washington, DC 20005. Duties include, but are not limited to; support groups, staff training, syringe exchange activities, and HIV Testing and Linkage services.
Responsibilities:
Daytime Service Center Support (30 hours/week)
● Assist DDSC staff in monitoring clients as they spend time in the drop-in center.
● Promote continuous communication between community partners on-site
● Facilitate Daily Support Groups
● Provide syringe exchange and distribute other harm reduction supplies
● Perform HIV/HCV Tests and linkage when necessary
● Inform clients of day center procedures, policy, and expectation.
● Safeguard patient privacy and confidentiality by always adhering to all HIPAA rules and regulations in order to protect Protected Health Information (PHI).
● Maintain an active inventory system for all supplies (harm reduction materials, condoms, syringes, naloxone..etc).
● Support risk and quality management by proactively communicating identified risks to your supervisor.
● Provide regular and ongoing feedback to Managers regarding flow and programmatic issues.
● Report data and/or statistics as required
Additional Responsibilities
● Attend weekly staff meetings, weekly one-on-one supervisions, monthly managers meetings, and monthly staff development days to share information and plan program work in a team environment.
● As part of a team, share in other organizational responsibilities as required including general administrative work and volunteer training

HIV prevention, treatment and care, and peer education and to disseminate information about HIPS programs to others
Minimum Qualifications and Skills Requirements

Under the guidance of the Director of Programs, the Coordinator will lead the implementation of outcomes and impact measurement for HIPS programs, coordinate and submit monthly reports to funders, and assist with proposal development. This technical role will involve acquiring, structuring, and enabling data so that it can be used in both reports and analysis.
Duties
Collect, synthesize, and analyze all program data at HIPS
Collect: Coordinate the monthly collection of program data across the organization, specifically for 10 separate funders.
Synthesize: Manage a centralized data collection by combining different data sets from across the organization. Manage the HIPS central data key, a collection of explanations of what we understand each data point to be asking.
Analyze: Examine data patterns in detail so as to identify trends, and then reflect on these trends in narrative form.

- Serve as the chief administrator of all data-related systems and accounts, including (but not limited to) HIPS internal databases.
- Coordinate training and technical support for all staff members for all data-related systems and accounts.
- Assist with proposal development and submission.

Qualifications and Skills Requirement
- Experience and familiarity with government databases including Redcap, ERA commons, and ASSIST.
- Must know how to use Google Suites, Microsoft Office, and Canva.
- Experience in training on data collection and computer literacy.
- Strong writing skills
- Familiarity with public health EBI’s and best practices.
- Experience with grant writing, particularly public health grants
- Passion for the mission of HIPS, including a strong commitment to working to improve the health and quality of life of people involved in street economies.
- Strong communication skills are essential.
- Ability to engage staff with all levels of computer literacy.
- Ability to maintain client records and information in an accurate, timely and confidential manner.
